Enrollment Process
Arizona law requires specific documentation to enroll your student. Review the requirements and prepare your documents before visiting. En Español ↓
Proof of Residency (A.R.S. §15-802(b))
Provide ONE of the following showing your current residential address:
- Utility bill (water, electric, gas, cable, internet)
- Property tax bill or mortgage statement
- Rental or lease agreement
- Arizona driver’s license or state ID
- Bank statement · paycheck stub · credit card statement
- Insurance policy or bill
- Vehicle registration · voter registration
- Court order or legal document
- Government-issued correspondence · public assistance documentation
- Tribal enrollment documentation
- Medical records or bills · employment records showing address
- Property deed · HOA documents
- School records from previous district
- Notarized affidavit of shared residence
Housing Instability Notice
Experiencing housing instability or homelessness and can’t provide the documentation above? Contact our front office immediately — under the McKinney-Vento Act we will work with you to enroll your student without delay. All information is kept confidential.
Proof of Age & Identity (A.R.S. §15-828)
Provide ONE of the following:
- Birth certificate (certified copy or original)
- Baptismal certificate with date of birth
- Passport or visa
- Hospital birth record
- Adoption records
- Court order establishing age
- Previous school records showing birthdate
Immunization Records (A.R.S. §15-872)
Arizona law requires proof of immunization for all students: documentation showing all required immunizations for their age group, or an exemption form (medical or personal belief). Students may be provisionally enrolled for up to 5 days while obtaining immunization records — contact the Arizona Department of Health Services or your healthcare provider for specific requirements.
Additional Required Information
- Emergency contacts — names, phone numbers, and addresses of at least two emergency contacts
- Previous school records — if transferring, request official transcripts be sent to Ridgeview College Prep
- Custody documentation — if applicable, court orders or custody agreements
- IEP or 504 Plan — if your student has special education needs or accommodations
- Medical information — critical medical conditions, allergies, or required medications

Expulsion from a Previous School (A.R.S. §15-184): Arizona law requires schools to ask whether a student has been expelled from another school district. Ridgeview may deny admission until the expulsion period has ended, admit the student on a probationary basis, or request additional information before deciding. If a student has been expelled, parents/guardians must disclose this during enrollment.
